Closed mouth   - ask patient to try to align the molars (not front teeth) as best they can, ie. bite so that back teeth are touching without clenching.  

Open mouth document the widest open mouth position possible biting on a syringe  as follows 

10 ml syringe- 10 mm
20 ml  syringe- 20 mm
30 ml syringe- 30 mm
Bite stick Green 33 mm  

Helical Supine. ( scan above the top of the TMJ through the mandibular condyle )- CLOSED Mouth
